Dental and Vision Plans

Dental and Vision plans you may be receiving as an employee are not part of your health insurance and do not continue automatically. If you receive these plans from a union Employee Benefit Fund (CSEA, UUP, DC-37, and some PEs) contact your fund for COBRA information; 60-day deadline applies. If you receive these plans through the State: Your employee dental and vision coverage will end 28 days after the last day of the last payroll period in which you worked. To continue under COBRA, contact the Employee Benefits Division within 60 days from the date coverage ends. For dental coverage, at the time you retire, you may continue under COBRA or choose the GHI Preferred Dental Plan. You automatically receive an enrollment form from GHI after you retire. If you choose COBRA, you also receive this form at the end of the 36-month COBRA period. If you do not receive the form, call your local GHI office or 1-800-947-0101 to request one.
