Dental and/or Vision Only Enrollee

Select Dental and/or Vision Only Enrollee under Choose Your Plan if you are not also enrolled in The Empire Plan or an HMO for health insurance through NYSHIP. If you receive dental and/or vision benefits in addition to health insurance through NYSHIP, be sure to select your health insurance plan (either The Empire Plan or an HMO) to receive complete information on your NYSHIP benefits.
