If you recently received a letter regarding claim payments made on behalf of an ineligible dependent and you believe that your dependent was eligible for coverage, it is very important that you submit acceptable documentation to prove the eligibility of your dependent as soon as possible. Once acceptable documentation is received, your dependent's coverage will be reinstated retroactive to February 1, 2009 and you will not be required to refund any claim payments.

  • If your dependent is ineligible and you have questions regarding Empire Plan claim payments, call 1-877-769-7447.
  • For UnitedHealthcare (medical, prescription drug or mental health/substance abuse claims) press prompt 7.
  • For Empire BlueCross (hospital claims), select prompt 8, then prompt 2.
  • If you are not the enrollee but will be acting on behalf of the enrollee, you must have  a HIPAA Authorization Form on file with the insurer(s) you will be contacting. Click here for the HIPAA Authorization Form.
