Special Amnesty Packet
Between April 3 – April 13, Budco Health Service Solutions will mail Special Amnesty Packets to the enrollee's home. Below is a sample of the packets. Enrollee should wait until they receive their packet from Budco to remove dependents during the Special Amnesty Period.
Note: the actual Eligibility Worksheet and Partial List of Documents you receive from Budco may look slightly different from the documents provided below. The version you receive is based on the benefits available to your particular group or union.

Enrollees who removed an ineligible dependent during the Amnesty Phase will be mailed this letter between June 22 – July 13.
