To reinstate dependents removed from coverage:
Click on the dependent type to see what documents must be submitted. Documentation must be submitted to your agency Health Benefits Administrator, usually located in your agency’s personnel office.
Make sure you submit a complete set of documents. See below to determine where to send your documents:
- If you are enrolled in NYSHIP through a local government, such as a county, city, town, school district, fire district or library district, submit documents to your agency Health Benefits Administrator, usually located in your agency’s personnel office.
- If you are enrolled in NYSHIP through a State agency or authority, submit documents to your agency Health Benefits Administrator, usually located in your agency’s personnel office.
- If you are a retiree of the State or an authority or are a former employee enrolled in NYSHIP through a State agency or authority, click here.
