Make sure you submit a complete set of documents, even if you previously submitted some documents to Budco. Submit documents to:

Mail Coversheet and documents for each ineligible dependent. You will receive a letter confirming the outcome of the review within three weeks.
If you have questions, call the NYS Department of Civil Service at 1-800-409-9059 Monday thru Friday EST from 8:00 a.m. to 5:00 p.m. Do not call to inquire about the status of documents you mailed.
