Classification Standard

BRIEF DESCRIPTION OF CLASS SERIES

          Medical Specialists are licensed physicians who have completed a residency training program in a medical specialty approved by the Accreditation Council for Graduate Medical Education (ACGME) or are certified in a medical specialty by a Member Board of the American Board of Medical Specialties (ABMS).  Incumbents perform physical examinations; diagnose medical conditions and prescribe treatments; collaborate with other health care providers and disciplines to achieve treatment goals and objectives; and provide a variety of medical care and treatment services for individuals* within a specialty area of practice in accordance with Article 131 of the Education Law and ABMS standards of care. 

          Positions are classified in various State agencies. 

*Individual refers to a person who is being treated or cared for, and is used in place of patient, client, or other nomenclature specific to an agency.

DISTINGUISHING CHARACTERISTICS

          RESIDENT PHYSICIAN: assists in providing medical care and treatment services within a specialty area of practice, typically under the direct supervision of a physician certified in the appropriate specialty.

          MEDICAL SPECIALIST 1: diagnoses, treats, prescribes, and provides specialized medical care and treatment services to assigned individuals, typically under the direction of a Medical Specialist 2. 

          MEDICAL SPECIALIST 2: independently diagnoses, treats, prescribes, and provides specialized medical care and treatment services to assigned individuals; provides clinical supervision and guidance to Resident Physicians, Medical Specialists 1, and/or other medical providers; and may manage and supervise medical services for an assigned facility, unit, or program.   

          MEDICAL SPECIALIST 3: serves as chief of service; manages, directs, and administers medical services for an assigned facility(s), unit(s), or program(s); develops, implements, and revises clinical policies and procedures; and directly supervises Medical Specialists, physicians, and/or other medical providers.  

RELATED CLASSES

Clinical Physicians are licensed physicians who provide general medical care and treatment services in accordance with Article 131 of the Education Law. 

           Psychiatrists are licensed physicians who have completed a training program in psychiatry approved by the American Board of Psychiatry and Neurology (ABPN) or are certified in psychiatry by ABPN, and provide psychiatric care and treatment services in accordance with Article 131 of the Education Law and ABPN standards of care. 

ILLUSTRATIVE DUTIES

          RESIDENT PHYSICIAN

Under the direct supervision of a physician certified in the appropriate specialty, assists in providing care and treatment services to individuals, which may include those described below for Medical Specialist 1.

          MEDICAL SPECIALIST 1

Provides a variety of specialized medical care and treatment services to assigned individuals. 

 

·       Obtains, reviews, updates, and records individual information, such as medical histories, test results, diagnoses, prescribed treatments, and individual responses; and clarifies information with individuals as needed. 

 

·       Conducts physical examinations; orders, performs, and interprets diagnostic tests and procedures; determines and/or confirms diagnoses; and treats a variety of specialized medical conditions. 

 

·       Prescribes and/or administers treatments, medications, therapies, vaccinations, and other specialized medical care to treat or prevent illness, disease, or injury; conducts follow-up examinations and monitors individuals’ condition, modifying treatment as needed. 

 

·       Collaborates with other health care professionals and disciplines to achieve treatment goals and objectives, and ensure continuity of care.

 

·       Explains and discusses procedures, test results, diagnoses, and prescribed treatments with individuals, families, and caregivers; and answers questions.  

 

·       Refers individuals to other health care providers based on diagnostic and treatment needs.

 

·       Participates in discharge planning and works collaboratively with other providers and disciplines as needed to facilitate medical follow-up after discharge.   

 

·       Maintains medical documentation consistent with agency, State, and federal requirements. 

 

Supervises individual care activities, and prescribes, orders, and oversees the administration of treatments. 

 

·       Consults with and advises other Medical Specialists, physicians, medical providers, and health care disciplines on diagnoses and treatment plans; and reviews and approves treatment recommendations.

 

·       Provides clinical supervision, guidance, and training to other medical professionals, nursing, and direct care staff; and monitors the coordination and implementation of care. 

 

·       Reviews individual records to confirm diagnoses, and/or ensure that treatments are appropriate for diagnoses and that records are properly maintained.

 

·       May assign individuals and individual care activities to other Medical Specialists, physicians, and/or medical providers; approve leave and track time and attendance; and complete performance evaluations. 

 

Functions as a member of an interdisciplinary treatment team, attends meetings, and serves on various committees and workgroups to improve individual services and outcomes. 

 

Assists in the development, implementation, and revision of clinical policies and procedures. 

 

May develop, implement, and/or supervise residency training programs and specialized health care and treatment education programs. 

 

MEDICAL SPECIALIST 2

 

In addition to performing any of the duties of a Medical Specialist 1 above, provides clinical supervision and guidance to Medical Specialists 1, physicians, and/or other medical providers.

 

May manage and supervise medical services for an assigned facility, unit, or program; perform the full range of supervisory duties; and develop, implement, and revise clinical policies and procedures.

 

MEDICAL SPECIALIST 3

 

In addition to performing any of the duties of a Medical Specialist 1 or 2 above, manages, directs, and administers medical services for a facility(s), unit(s), or program(s).

 

Develops, implements, and revises clinical policies and procedures.

 

Directly supervises Medical Specialists, physicians, and/or other medical providers; and performs the full range of supervisory duties. 

 

MINIMUM QUALIFICATIONS

          RESIDENT PHYSICIAN

Non-Competitive: Admission to a residency training program in a medical specialty approved by the ACGME or ABPN.

          MEDICAL SPECIALIST 1

Open Competitive: Possession of a license and current registration to practice medicine in New York State, and successful completion of a residency training program in a medical specialty approved by the ACGME.

          MEDICAL SPECIALIST 2

Open Competitive: Possession of a license and current registration to practice medicine in New York State, and certification in a medical specialty by a Member Board of the ABMS. 

          MEDICAL SPECIALIST 3

Promotion: One year of service as a Medical Specialist 2, license and current registration to practice medicine in New York State, and certification in a medical specialty by a Member Board of the ABMS.

Open Competitive: Possession of a license and current registration to practice medicine in New York State, certification in a medical specialty by a Member Board of the ABMS, and one year of post-certification experience within the medical specialty.
