Health Plan Choices
The health plan options available to you are based on your parent's eligibility. It is important you know this information before proceeding further.
If your parent is eligible for NYSHIP benefits and is a member of one of the following groups:
- Civil Service Employees Association (CSEA)
- Council 82
- Management/Confidential (M/C); Legislature
- New York State Corrections Officers and Police Benevolent Association (NYSCOPBA)
- Unified Court System (UCS)
- Public Employees Federation (PEF) or
- Agency Law Enforcement Services Unit (ALESU)

If your parent is eligible for NYSHIP and is a member of:
- District Council 37 (DC-37)
- Police Benevolent Association (PBA)
- Police Investigators Association (PIA)
- United University Professions (UUP)
