Reporting On....Women's Health Issues
This special section, "Reporting on... .Women's Health Issues," includes general and Empire Plan specific information on health issues that are important to women. Topics include pap tests, thyroid disease, mammography and breast self-examination.
"Reporting On Women's Health Issues" will be available as a stand-alone in December. We will send you a small supply. You will be able to order the stand-alone, at that time, using the Publication Supply Order Form. (We will update the order forms as the"Reporting On. . . Women's Health Issues" are distributed.)
Distribution of Empire Plan Reports
The Reports will mail to enrollee homes in October. We will send you a supply of the Reports equal to 20 percent of your enrollment as we mail each version. We will send you a supply of the Retiree version equal to 10 percent of your enrollment when it is mailed.
We have attached a Publications Supply Order Form to this memo. All of the Reports are listed, but they will not be available right away. Please wait until you receive your supply of a specific group before you place an order.
