We will soon be mailing the March 1998 Empire Plan Report for employees and retirees enrolled through Participating Agencies to enrollee homes by first class mail. All Participating Agency enrollees will receive the same version of this Report.

The four-page Empire Plan Report includes the following:

Distribution of Empire Plan Report

The Report will mail to employee and retiree homes beginning in mid-March. We will send you a supply of the Report equal to 20 percent of your enrollment.

We have attached a Publications Supply Order Form to this memo. The Report is listed, but it will not be available right away. Please wait until you receive your supply before you place an order.
