The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

We will soon be sending a group-specific Special Mailing to enrollees that will include the following:

Reporting on... Using Empire Plan Network Benefits (1999)

This group-specific four-page report explains how to use the Empire Plan provider networks in order to obtain maximum benefits under the Empire Plan.

1999 Empire Plan at a Glance

Includes 1999 deductible and coinsurance amounts.

1999 Copayment Card

A quick reference pocket card with Empire Plan copayments specific for each group.

UnitedHealthcare Reference Card

A quick reference pocket card with instructions on how to use the UnitedHealthcare telephone system.

Distribution

The Special Mailing will mail to enrollee homes in March. Please give the Special Mailing to new employees as part of their enrollment packages. We will send you a supply equal to 10 percent of your enrollment as we mail each version.

We have enclosed a Publications Supply Order Form with this memo. All of the Special Mailings are listed, but they will not be available right away. Please wait until you receive your supply of a specific group before you place an order.

We will also send you a 10% supply of the stand-alone Empire Plan At A Glance.

These publications replace the 1998 versions of the Special Mailing and the Empire Plan at a Glance. Please recycle the 1998 versions and replace with the new supply we are sending you for new hires.
