We are happy to announce that new Empire Plan Participating Provider Directories will begin mailing to Empire Plan enrollees in late November. Here are the details:
Directory Contents
The 1999 directories list Empire Plan Medical/Surgical, Managed Physical Medicine Program and Mental Health/Substance Abuse providers. They also include Ambulatory Surgical Centers, Cardiac Rehabilitation Centers, Urgent Care Centers and Laboratories.
Directory Versions
Directories are being produced in four versions:
- Upstate New York (includes Massachusetts, Pennsylvania and Vermont)
- Downstate New York (includes Connecticut and New Jersey
- Florida
- Individual State versions: The individual state versions are for enrollees in the states not included in the Upstate, Downstate and Florida books. The individual State directories will be mailed to enrollees in late December-January.
Distribution/Agency Orders--Details
The Florida, Upstate and Downstate directories will begin mailing to enrollees in November and continue through December. By early December, we will send you a supply of directories for your region equal to five percent of your enrollment. You will also receive two copies of the Florida directory plus two copies of the other New York regional directory. The versions will come to you as separate mailings.
Some larger agencies with employees in both the Upstate and Downstate areas will receive split shipments based on employee locations. We have contacted these agencies.
The Upstate, Downstate and Florida directories are listed on a new Publications Supply Request form on the back of this memo. You may order additional directories after you receive your shipment. And, you may order other regions if you need them.
When the Individual State versions are printed, we will include those on an updated order form. We are printing small quantities of most states, so we ask that you order the individual versions on an as needed basis. We will send you an order form for the individual states with a future memo. Please order only what you need.

Directories for employees who change options
The labels for enrollee mailings were drawn the first week of November. New employees and employees who change into the Empire Plan will need a directory.
