We will soon be sending a group-specific Special Mailing to enrollees that will include the following:
Reporting on... Using Empire Plan Network Benefits
This group-specific four-page report explains how to use the Empire Plan provider networks in order to obtain maximum benefits under the Empire Plan.
1998 Empire Plan at a Glance (group specific)
Includes 1998 deductible and coinsurance amounts.
Copayment Card
A quick reference pocket card with Empire Plan copayments specific for each group.
UnitedHealthcare Reference Card
A quick reference pocket card with instructions on how to use the UnitedHealthcare telephone system.
Distribution
The Special Mailing will mail to enrollee homes in mid-June. We will send you a supply equal to 10 percent of your enrollment as we mail each version. New York State and Participating Employer agencies will also receive a separate retiree version when it is mailed. These can be added to the packages you give to employees at retirement.
Special note to Participating Agencies: If mail for your enrollees is returned to you as undeliverable, please call your processor with questions.
