We are happy to announce that new Empire Plan Participating Provider Directories were mailed to out-of-state employees, retirees and other enrollees in January. Here are the details:
Directory Contents
The December 1999 directories list Empire Plan Medical/Surgical, Managed Physical Medicine Program and Mental Health/Substance Abuse providers. They also include Ambulatory Surgical Centers, Cardiac Rehabilitation Centers, Urgent Care Centers and Laboratories.
Directory Versions
Out-of-state directories are being produced for all states except Massachusetts, Pennsylvania and Vermont which are included in the Upstate New York version; Connecticut and New Jersey which are included in the Downstate New York version and Florida (see PA99-13).
Distribution/Agency Orders--Details
Attached is a special order form for the out-of-state directories. Please save this form and photocopy as needed. We ask that agencies do not order a supply of each directory, as quantities are limited. Please order directories only as you need them to give to enrollees:
- who are moving.
- who are traveling to that state.
- who retire.
- with dependents, such as college students.
Even for these enrollees, it is better to visit our Web site at http://www.cs.state.ny.us and print an updated copy. Providers change and the Web site is updated weekly.
