Description:

Mid-Hudson Forensic Psychiatric Center serves approximately 285 men and women who have mental health concerns as well as criminal justice involvement in a hospital setting. We are a state operated facility, located in New Hampton, NY, near Middletown and Goshen, in Orange County.
Pharmacy Interns will participate in patient centered care daily.
Activities of the rotation:
* maintain the confidentiality of patient information
* demonstrate in depth knowledge of the medications encountered most frequently on the service including indication, route of administration, compatibility, pharmacokinetics, dose, adverse effect profile and monitoring parameters.
* Collect accurate and comprehensive information from the patient chart.
* Apply knowledge of the pathophysiology of disease states to assess drug therapy.
* Recommend changes in drug therapy based on product information, changes in patient medical status and published literature. (evidenced based medicine, clinical studies).
* Identify patient specific drug related problems and communicate (verbal and written) alternative strategies to the prescriber to correct or prevent the problem.
* Respond to medication information requests from medical team members and preceptor. (include references).
* Provide medication reconciliation services.
