Provides all New York State departments and agencies medical examination services mandated by Civil Service Law.
Overview
Employee Health Service (EHS) offers medical examination services, including pre-employment, fitness-for-duty / return-to-work, and occupational health examinations, for all New York State departments and agencies. EHS provides agencies with a determination of an individual's ability to safely perform the duties of a State job and assists agencies to fulfill mandated medical monitoring requirements. In addition, EHS Nurses perform occupational health screening and immunization services throughout the State.
Medical Records Requests
If you received services from EHS and wish to have a copy of your medical records, complete form EHS-742, Authorization for Release and Disclosure of Medical Information and send it to EHS.

Pre-placement Physicals
Numerous State job titles have physical and medical requirements that require a pre-placement physical examination. The examination announcement for the job title will indicate under "Qualifications for appointment" whether physical/medical requirements apply to that job title. A list of job titles that have physical/medical requirements can also be found here.
Your prospective agency will schedule your pre-placement physical examination once you are given an offer of employment. Depending on your agency and/or job title, your physical examination will be scheduled at one of our EHS medical examination centers, at one of our vendor locations, or at your prospective agency's job site. Your prospective agency will notify you of the time and location of your appointment. Please notify your prospective agency as soon as possible if you need to cancel or change your appointment.

Occupational Health
EHS provides occupational health examinations and medical monitoring pursuant to the
Public Employee Safety & Health Act.
Your agency will notify you when these examinations are required and will schedule your appointment at one of our
EHS medical examination centers, at one of our vendor locations, or at your agency's job site.
Your agency will notify you of the time and location of your appointment. Please notify your agency as soon as possible if you need to cancel or change your appointment.
Reinstatement Requests
Certain employees who were terminated as a result of a physical or mental disability have the right to a reinstatement medical evaluation to determine their ability to perform the duties of the position from which they were terminated. Reinstatement medical examinations are conducted at the EHS medical examination center in Cohoes. A former employee seeking reinstatement must complete form EHS-705, Reinstatement Examination Request and send it to EHS to request a reinstatement physical examination.
