New York State Active Employee (NY)

I am an employee of the State of New York, working for an agency within the New York State Government structure.

Participating
Employer (PE)

I am an employee of a Participating Employer of New York State, working for a quasi-governmental agency in NY maintained and financed from special administrative funds.

Participating
Agency (PA)

I am an employee of a Participating Agency of New York State, working for a local government agency in New York State including a County, City, Town, Village, School District or other municipal entity.

Student Employee Health Plan (SEHP)

I am a Graduate Student enrolled in the Student Employee Health Plan (SEHP).

I am a retiree
