Information About Measles and Travel
The New York State Department of Health has released guidance for travel and measles. Click here to read the full release.
The Empire Plan covers the measles, mumps and rubella (MMR) vaccine with no out-of-pocket cost when you use a network medical provider or for adults at a network vaccine pharmacy.
If you are unsure if you are vaccinated, try to find your vaccination records, or consult with your health care provider. Your provider or local health department can help you decide if you need an MMR vaccination.

Your new Empire Plan Certificate and the accompanying Certificate Amendments are now available. Both the Certificate and Amendments are effective July 1, 2023; the Amendments detail benefits changes since your last Certificate was published.
A printed copy of the Empire Plan Certificate is being mailed to your home address. Keep it with your records and recycle any previous versions you may have. The Certificate Amendments have been incorporated into the new version; they are only available online as a reference for you.
