We are pleased to announce the publication of the 2026 Empire Plan Preventive Care Coverage Guide (PCCG).
The PCCG includes a summary of current federal Patient Protection and Affordable Care Act (PPACA) coverage requirements and information on preventive versus routine or diagnostic services. Encourage enrollees to use this guide together with their Empire Plan Certificate, Empire Plan Certificate Amendments, Empire Plan Report and Empire Plan At A Glance.
Distribution and Access
We will mail and email the PCCG to enrollees in June. The file for this mailing was pulled from NYBEAS on June 24, 2026, so any enrollee not enrolled in The Empire Plan on NYBEAS as of this date should be provided with a copy.
This publication may also be accessed on our website at nyship.ny.gov.
Ordering Publications
In the coming weeks, copies of the PCCG may be ordered on HBA Online under “Publications & Forms” using the publication code AL2098.
