We are pleased to announce that the new version of the publication Reporting On Diabetes is at the printer and will be shipped to agencies soon.

The April 2012 version replaces the July 2010 version and includes valuable information about The Empire Plan's Home Care Advocacy Program (HCAP) and diabetes education and self-care. Please recycle any copies you may have of the July 2010 version.

Distribution
We will send you five copies of this publication in mid April, but you can order as many as you need on HBA Online. It will not be mailed to enrollees' homes.
