The communications schedule through March 1994 includes several pieces:

1. PARTICIPATING PROVIDER DIRECTORIES

The 11 versions of the directories are now in production and will be shipped to you late February. We will ship you 115 percent of your enrollment based on your agency's zip code as a start-up supply. Once all the versions are printed, we will send you a complete set of the 11 versions. Attachment A is a one-time order form to get you the additional versions you need as a start-up supply. You may fax this order to the Communications Unit (518) 457-2494 or mail to the address above. Order only what you need up to 5% of your enrollment.
For later orders, please use the updated publications supply request form which is attached. If you have questions about ordering directories, please call Communications at (518) 457-7577.

2. MARCH REPORTS

Empire Plan: We are developing March Empire Plan Reports in group-specific versions. The reports will include information on the Ambulatory Surgical Centers now participating in the Empire Plan, the MetLife phone system and copayments. This issue will also announce HealthCall's high risk pregnancy program.
We will send you more information on the contents and mailing dates closer to the printing dates.

3. EMPIRE PLAN PRESCRIPTION DRUG PROGRAM MATERIALS

The November 1993 Empire Plan Report introducing ValueRx, the new prescription drug carrier effective January 1, 1994, was mailed to enrollees and agencies. A new ValueRx/AccuScript mail service envelope is inserted in the Report. If you need more copies, please use your publications order form. (See HBA memo PA 93-13 and attachment B.)
ValueRx sent you extra ValueRx/AccuScript mail order envelopes and claim forms. These supplies replace the NRx mail service envelope and PAID Prescriptions claim form. For more envelopes or claim forms, call ValueRx at 1-800-964-1888.
Please recycle obsolete prescription drug materials, including the Directories of Participating Maintenance Drug Program Pharmacies.

4. ATTACHED: REVISED PUBLICATION SUPPLY REQUEST FORMS

The attached Publication Supply Request forms are updated to allow you to order recent publications. Please photocopy the form as needed.
Thank you for letting us know that you like the new ordering system. To be sure that the system works for you, do not write anything in the borders or white space on the form. If you are unsure how to order the publication you want, ask your processor or call the Communications Unit at (518) 457-7577.

5. PARTICIPATING AGENCY POLYWRAP PACKAGES

Please remove the Directory of Participating Maintenance Drug Program Pharmacies and NRx envelope from the Participating Agency polywrap packages.
We are preparing new packages that you may order as you need.
