In our January 28, 1994 Communications Update memo (PA94-01), we promised to send you more information on the contents and mailing dates of the March Reports. Here is what's coming:

Empire Plan Reports:

Group-specific versions of the March 1994 Empire Plan Reports will include information on:

Ambulatory Surgical Centers now participating in the Empire Plan
MetLife Phone System
New Blue Cross telephone number
Deductibles and Copayments
Health Insurance News and Reminders
Filing Claims with MetLife
Family and Medical Leave Act.

A special insert, Reporting On... Healthy Babies, introduces HealthCall's new pregnancy program.

You will be receiving a supply of Reports equal to 120 percent of your enrollment. Please distribute them to both your active and retired employees right away. Some Reports are being returned to us as undeliverable, often for inadequate postage. When you mail the Reports to your retirees, please:

  1. fold the Report in third to fit inside a #10 business size envelope with 29 cents in postage for first class mailing, or
  2. place the Report flat in an 8 1/2 by 11 inch manila envelope with 52 cents in postage for first class mailing, or
  3. fold the Report in half, close the long edge with a piece of tape and cover the postage permit with 29 cents in postage for first class mailing, or
  4. trifold the Report, close the long edge with a piece of tape and cover the postage permit with 29 cents in postage for first class mailing.

Participating Provider Directories:

We are continuing to fill your re-orders for Participating Provider Directories. As of March 31, please use only the attached form to order additional Participating Provider Directories and for your other publication needs.

Publications Supply Request Form:

The attached Publication Supply Request Form is updated to allow you to order recent publications. Do not write anything in the borders or white space on the form. If you are unsure how to order the publication you want, ask your processor or call the Communications Unit at (518) 457-7577. Please photocopy the form as needed.
