We are happy to send you an advance copy of the January 1995 Empire Plan Report for Participating Agencies. This version explains:
- Expanded Basic Medical benefits for routine pediatric care
- Durable medical equipment and supplies covered under HCAP
- Domestic partner coverage, an optional benefit for Participating Agencies
- Emergency room visits
- Medicare coverage under age 65 and the Medicare Part B premium
- 1995 annual deductible and coinsurance maximum
- Premium payment while on voluntary military leave
- Empire Plan claims deadlines
- Metropolitan's new name
Participating Agency employees and retirees will receive the final version of the January Reports by first class mail at their home addresses in February. You will receive a supply equal to 20 percent of your enrollment.
