We have prepared the December 1995 Empire Plan Report and NYSHIP Changes, Empire Plan Certificate Amendments and Prescription Drug Certificate for Active Employees, Retirees, Vestees and Dependent Survivors and their Dependents enrolled through Participating Agencies.

The Report explains benefit changes which include:

Effective January 1, 1996

  • Managed Physical Medicine Program
  • Domestic Partner Eligibility
  • HealthCall changes regarding prospective procedure review and specialist consultant evaluation
  • $25 hospital outpatient copayment
  • Basic Medical benefits for routine health exams and ambulance service
  • Mental Health and Substance Abuse Program
  • Prescription Drug Program

Changes/Amendments to the 1995 NYSHIP General Information Book and Empire Plan Certificate and the Empire Plan Prescription Drug Certificate in a book insert.

Distribution

Enrollees will receive the December 1995 Report and Certificate appropriate for their option by first class mail at their home addresses in January. You will receive a supply equal to 20 percent of your enrollment of each Report. If the Report is not deliverable to the enrollee's address, we will return the Reports to you for timely distribution to the enrollee.

Publications Supply Request Form: The attached Publications Supply Request Form is updated to allow you to order recent publications. Please do not write anything in the borders or white space on the form. Please do not use old copies of this form. Please photocopy the form, as needed.
