SUBJECT: Empire Plan Participating Provider Directory--HBA Reference Copies

We are pleased to announce that we are printing an updated Empire Plan Participating Provider Directory for your use. The directory is for use as an agency reference, not for distribution to enrollees. Print quantities will be very small. In order to further reduce costs, we have combined the regional directories into one version. The directory will list medical and Managed Physical Medicine Program providers.
As you know, providers change. Please continue to encourage employees who ask about participating providers to check with the provider directly or to call MetraHealth at 1-800-942-4640.
Distribution
The Directories will begin to ship to agencies in late November. We will send you a supply of the Directories equal to one for every 250 enrollees with a minimum of one to each agency.
If you have district or field offices and need additional copies right away, please use the special order form below. For later orders, we will have a limited supply in surplus, and you may order extra copies using the attached Publications Supply Request Form.
