This five-page insert is a concise guide to Empire Plan benefits. The Empire Plan at a Glance is a useful publication for new employees, conferences and health fairs. Encourage employees to use this guide together with an Empire Plan Certificate.
The Empire Plan at a Glance will be available as a stand-alone in January. We will send you a small supply. You will be able to order the guides using the Publication Supply Order Form. (We will update the order forms as the guides are distributed.)
Distribution of Empire Plan Reports
The Reports will mail to enrollee homes in December. We will send you a supply of the Reports equal to 20 percent of your enrollment.
