We are mailing the December 1996 Empire Plan Reports for Participating Agencies with Core Only, Core Plus Medical Enhancement and Core Plus Medical and Psychiatric Enhancements in late December.
The Reports include Empire Plan information, certificate amendments, NYSHIP changes and The Empire Plan at a Glance.

Telephone Card Strips
We sent you phone card strips (EP-TEL-Rx-4/96) in July. If you need more, you may order them by using the Publication Supply Request Form on the back of this memo. We will include the card strips in a future Report.
Distribution of Empire Plan Reports
We will mail the December 1996 Reports to enrollee homes in late December.
We will send you a supply of the Reports equal to 20 percent of your enrollment.
The Publications Supply Request Form on the back of this memo has been updated so you may order the new Empire Plan Reports. Please do not order additional copies until you receive your initial supply. Do not use old copies of this form or write anything in the borders or white space. Photocopy as needed.
