A review of the Blue Cross database has shown approxirnately 500 enrollee names with no addresses. If any of these enrollees are from your agency, we have attached a list with enrollee name(s) and ID number. Please contact the enrollee for a current address and make the appropriate change by submitting form PS 503.1 for all employees listed.
If there are no missing addresses for your agency, there will be no attachments with this memo.
