We are pleased to announce that the 3 versions of the 2000 Empire Plan At a Glance for employees of Participating Agencies with CORE Only, CORE Plus Medical Enhancement or CORE Plus Medical and Psychiatric Enhancements benefit packages are being printed and will be sent to you later in December.

The appropriate version of the Empire Plan At a Glance, a concise guide to plan benefits your agency has chosen, will mail to enrollee homes in 2000. The Empire Plan At a Glance is a useful publication for new employees, conferences and health fairs. Encourage employees to use this guide together with their Empire Plan Certificate.
Distribution
We are sending you a supply of the appropriate Empire Plan At a Glance equal to 20 percent of your enrollment. Please destroy old copies of these forms when the 2000 Empire Plan At A Glance flyers are received.
The attached Publications Supply Request Form is updated to allow you to order the 2000 Empire Plan At a Glance for the benefit plan your agency has chosen. Please wait until you receive your supply before you place an order. Please do not use old copies of this form or write anything in the borders or white space. Photocopy as needed.
