SUBJECT: Excelsior Benefit card reissue

Excelsior benefit cards are being reissued to all Excelsior Plan enrollees and dependents due to a new Prescription Drug carrier. A carrier number was removed from the back of the card. No new number was needed.

Families will receive up to two card carriers with two cards in an envelope, depending on the number of dependents. Individual coverage enrollees will receive one benefit card and one card marked Void as the card carriers come in twos.

The new Excelsior cards were mailed to enrollee homes December 20, 2013. The files were drawn on December 11, 2013.
