SUBJECT: NYSHIP General Information Book for Active Employees of Participating Employers enrolled in HMOs

We are happy to announce that the August 1, 2001 NYSHlP General Information Book for Active Employees of Participating Employers enrolled in NYSHIP-approved HMOs and for their enrolled dependents and for COBRA enrollees with their benefits is at the printer and will be mailed in August.
We are planning to issue a new book/certificate for active employees of Participating Employers in the Empire Plan later this year.
Distribution
As we mail these books to employees homes we will send you a supply equal to 10 percent of your enrollment. Please be sure to give a copy of this book to each new hire who enrolls in an HMO.
Reordering
The new books replace the 1995 General Information Book for Active Employees of Participating Employers in HMOs and all NYSHIP HMO Reports updating that book. If you have a surplus of these materials, please follow your local recycling regulations.
We have attached an updated Publications Supply Order Form to this memo. Or, instead of using the form you may order on PA-MARKET. From the homepage, open Publications and Forms, then open NYSHIP Publication Order Forms. Here you may select a group, then "Order Online" or "Print or View Form" and follow instructions.
