Effective January 1,1996, the monthly premium rates for the New York State Employees Dental and Vision Insurance Programs were changed for all groups.
Enclosed is a supply of the New York State Dental Insurance Program's 1996 Monthly Coverage report (PS-802). Please destroy any remaining inventory of outdated PS-802 forms.
