We are happy to announce the March 1996 Empire Plan Report for employees enrolled through Participating Employers. The Report includes information on:

• Managed Physical Medicine Program
• 1996 Annual Deductible and Coinsurance Maximum
• Claims deadlines and new claim form
• HealthCall and Magnetic Resonance Imaging (MRI)
• Keeping Coverage Up to Date
• Ambulatory Surgical Centers
• 1996 Pre-Retirement Seminars

A NYSHIP General Information Book and Empire Plan Certificate inserted within the Report includes:

• NYSHIP changes
• Empire Plan Certificate Amendments
• Empire Plan Prescription Drug Program Certificate

We've also inserted an Empire Plan Telephone Numbers card strip. We will send you 10 extra cards for new enrollees.

Distribution
Enrollees will receive the Report at home by third class guaranteed return mail. We will send you a supply equal to 20 percent of your enrollment. If you need additional copies, please use the Publications Supply Request Form on the back of this memo.
