We will soon be mailing the March 1998 Empire Plan Report for employees and retirees enrolled through Participating Employers to enrollee homes by first class mail.

The four-page Empire Plan Report includes the following:

Distribution ofEmpire Plan Report
The Report will mail to enrollee homes beginning in mid-March. We will send you a supply of the Report equal to 20 percent of your enrollment.

HMO Reports
HMO Reports are being prepared for all groups. We will send you a separate memo with details.
