We are happy to announce NYSHIP Health Maintenance Reports for March and April 1998. There are two versions:

• Active employees of the State of New York; Legislature; Unified Court System; Participating Employers (April)
• Retirees (March)

The Reports include information on:

• Chiropractic legislation
• Mastectomy length of stay and reconstructive surgery legislation
• Medicare coordination period for end stage renal disease
• Planning to retire
• Internet

We will mail each Report to enrollee homes beginning in mid-March. We will send you a supply of the Actives version equal to 20 percent of your enrollment. We will send you a supply of the Retiree Report equal to 10 percent of your active enrollment to give to new retirees.

If you need additional materials, we have supplied a Publications Supply Request Form that lists the above Reports. Please do not order any Reports until you have received your initial supply. Please do not use old copies of this form or write anything in the borders or white space. Photocopy as needed. When you fax your order form for these publications, you do not need to send a cover sheet, provided you fill out all necessary information. Please take the time to fill out your order form completely so that if we have questions we can call you.
