In response to questions received by the Employee Benefits Division regarding National Medical Support Notices, we have developed some resources to facilitate agency responses. Attached are two documents: 1) A carrier contact list and 2) a Frequently Asked Questions (FAQ) document.

The carrier contact list may be used instead of completing the Plan Administrator Addendum. Merely check the appropriate carrier box(es) that apply to the enrollee’s circumstances and include this with the relevant response documentation as outlined in the March 26, 2004 HBA memo regarding National Medical Support Notice Guidelines.

The FAQ provide answers to issues commonly raised by agencies. Questions regarding these documents may be forwarded to your agency processor.
