The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

SUBJECT: July 2015 SEHP Benefit card distribution

SEHP benefit cards will be mailed to all enrollees and dependents beginning Tuesday, July 21, 2015.
This is the annual distribution to all SEHP enrollees and their dependents. The only change for this reissue is the expiration date of August 31, 2016.
The existing card has the current copayments. Please remind enrollees to destroy the existing card when changing to the newly issued one.
The files were pulled from NYBEAS on July 15, 2015. Any employee added to NYBEAS after that date will receive a card in our normal weekly card mailing for new SEHP enrollees.
