SUBJECT: Reporting On Asthma and Reporting On Diabetes
DATE: May 10, 2007
We are happy to announce that the May 2007 Reporting On Asthma and Reporting On Diabetes
are at the printer. These newly revised publications replace the July 2006 and March 2006
versions respectively.
Both publications include information about UnitedHealthcare’s new Disease Management
Program. The Program combines the individual programs for asthma, diabetes and
cardiovascular disease into a single program, and adds two new conditions: congestive heart
failure and chronic obstructive pulmonary disease. If an enrollee is identified as someone with
one of these conditions that would benefit from disease management, UnitedHealthcare will
contact the enrollee to participate in the new program.
Distribution
We will not be mailing these publications to enrollee homes. We will send you a supply of each
equal to 3 percent of your enrollment for use at health fairs, distribution to new hires and retirees
and general use.
