We are happy to send the revised September 2007 Reporting On HCAP. This publication replaces the December 2005 version. If you still have the old publication on your shelves, please remove and recycle it.
This publication explains The Empire Plan Home Care Advocacy Program (HCAP) and is for enrollees who need home care services, durable medical equipment and certain supplies. The revisions are minor, mostly affecting the name of the diabetic supplies pharmacy. Only the name has changed; the number is the same.
Distribution
We will not be mailing this publication to enrollee homes. We have enclosed one copy of this new publication (AL0785 RO HCAP REV September 07), which is popular at health fairs and with new enrollees. If you would like additional copies and you use the convenient online ordering system on HBA Online, the form is now updated with the new publication. If you do not have access to the internet, we have enclosed a paper order form that you may use until we update your printed forms later this fall.
