SUBJECT: New NYS Vision Plan Booklets for Contract Affected Employees Represented by Council 82 (NU91) and NYSCOPBA (PE-NU01, NU21)

As a result of the settlement of contracts that provide for the October 1, 2007 implementation of Vision Plan benefit changes, EyeMed Vision Care will soon be sending you a small supply of new NYS Vision Plan booklets for Contract Affected Security Supervisors Unit enrollees represented by Council 82 and Contract Affected Security Supervisors Unit enrollees represented by NYSCOPBA in the Negotiating Units (NU) shown above.

The new booklets have a revised cover page and information on a newly implemented laser vision correction benefit.

Enclosed with the booklets will be a cover letter and a NYS Vision Plan Publications Supply Request Form with instructions for requesting additional booklets from EyeMed Vision Care. 

Please be advised that current affected enrollees will have new booklets sent directly to their homes.  Please reserve the supply of booklets you receive for new employees.
