We are pleased to announce the 2008 Empire Plan At A Glance for Council 82, Courts, CSEA, DC-37, M/C; Legislature, NYSCOPBA, PBA Supervisors and Troopers, PEF, PIA, UUP, Retirees, Participating Employers and Participating Agencies Core Plus Enhancements and Core Only. The Empire Plan At A Glance, a guide to plan benefits, is a useful publication for new employees, conferences and health fairs. Encourage employees to use this guide together with their Empire Plan Certificate.
The 2008 Empire Plan At A Glance contains a Special Prescription Drug Report that contains important information regarding the new prescription drug program vendors; UnitedHealthcare and Medco Health Solutins, and the 2008 Empire Plan Preferred Drug List. See HBA Memo NY07-40, PE07-29, PA07-23 for information.
Distribution
These publications will be mailed to Empire Plan enrollee homes beginning in mid-December. We will send you, in separate shipments, a supply of each publication equal to 3 percent of a dropshipment of the 2008 Preferred Drug List that was delivered to agencies in November, Empire Plan enrollment for new hires or for those enrollees changing their option to The Empire Plan.

Or, instead of faxing the form, you may order on HBA Online. Open Publications & Forms, then open NYSHIP Publication Order Forms and choose the appropriate group.
