SUBJECT: MHSA Stickers for 2008 Participating Provider Directories

PPD Cover with MHSA stickerPlease find enclosed copies of stickers for your inventory supply of Empire Plan 2008 Participating Provider Directories. A single sticker should be adhered to the cover of each directory you provide to an enrollee on or after January 1, 2009. Please do not cover any text on the cover with the sticker, (see Downstate, New York example, right).

You may also provide stickers to enrollees who request them for copies they’ve received at their homes. Additional stickers are available to order using the enclosed Special Order Form.

This sticker is necessary due to the change in the Mental Health and Substance Abuse (MHSA) vendor, from GHI/ValueOptions to UnitedHealthcare/OptumHealth Behavioral Solutions, after the directory’s printing. We apologize for any confusion this change may cause. The 2009 directories are expected to be available during the late summer, 2009.
