Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

SUBJECT: July 2018 SEHP Benefit Card Annual Distribution

The 2018-2019 SEHP benefit cards will be mailed to all enrollees and dependents on July 20, 2018.
This is the annual distribution to all SEHP enrollees and their dependents. The only change for this reissue is the expiration date of August 31, 2019. Please remind enrollees to destroy the existing card when changing to the newly issued one.
The card file was pulled from NYBEAS on July 11, 2018. Any enrollee added to NYBEAS after that date will receive a card in our normal weekly card mailing for new SEHP enrollees.
Note: The new SEHP General Information Book is nearly completed and we will be mailing copies to SEHP enrollees and agencies later this summer. Please be sure to confirm and update SEHP enrollee addresses to ensure proper and timely delivery of these materials.
