The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

SUBJECT: 2021-2022 SEHP Benefit Card Distribution

The 2021-2022 SEHP benefit cards were mailed to all enrollees and dependents on July 23, 2021. This is the annual distribution to all SEHP enrollees and their dependents. The expiration date on the cards is August 31, 2022. Please remind enrollees to securely destroy the existing card when changing to the new version.
The card file was pulled from NYBEAS on July 12, 2021. Any enrollee added to NYBEAS after that date will receive a card in the normal weekly card mailing for new SEHP enrollees.
