A General Information Book and Empire Plan Certificate inserted within the Report includes:

• NYSHIP changes
• Empire Plan Certificate Amendments
• Empire Plan Prescription Drug Program Certificate

We've also inserted an Empire Plan Telephone Numbers card strip.
