Notice for Posting: On the reverse side of this memo is a notice for you to copy and post. If you are a Health Benefits Administrator in an agency with regional offices, please distribute notices to the regional locations as well. This notice should be posted in a convenient location until March 31, 1996.
Empire Plan Claims Deadlines
Metropolitan claims for the Empire Plan Basic Medical Program, for the Managed Physical Medicine Program and for non-network mental health and substance abuse services provided in 1995 must be submitted by March 31, 1996 (90 days after the end of the calendar year).
If the Empire Plan is the secondary insurer, Metropolitan claims must be submitted by March 31, 1996 or within 90 days after your primary health insurance plan processes your claim, whichever is later. Ask your agency Health Benefits Administrator for claim forms or call MetraHealth Service Corp. at 1-800-942-4640.

If you are covered by the Empire Plan Prescription Drug Program, you must submit claims for prescriptions filled in 1995 by March 31, 1996 (90 days after the end of the calendar year). Call ValueRx at 1-800-964-1888 for claim forms.
