SUBJECT: New Procedure for Processing M/C Life Insurance Retirement Transactions
DATE: May 16, 1996
Effective the date of this letter, the Employee Benefits Division instituted a new procedure for processing transactions for employees who retire and have M/C Life Insurance coverage. The procedure is designed to inform the prospective retiree of options available concerning their M/C Life Insurance coverage and to reduce the amount of intervention required by the agency's Benefits Administrator.
When an Enrollee Retires
There is a new procedure for processing these transactions.
- Give the enrollee the M/C Life Insurance instructions and retirement package including:
a) M/C Life Insurance New Retiree Information form PS-931 (5/96)
b) M/C Life Insurance Transition to Retirement Notice PS-932 (5/96)
c) Monthly Rate Schedule for Management/Confidential Group Life Insurance PS-915.1 (8/95) - The enrollee completes the M/C Life Insurance Transition to Retirement Notice (PS-932) and returns it to the agency Benefits Administrator prior to the enrollee's retirement date.
- Review the form for completeness and accuracy. Complete and sign Section II of the form.
- Give a completed copy of the M/C Life Insurance Transition to Retirement Notice to the enrollee and retain a copy for the agency.
- Forward the completed original M/C Life Insurance Transition to Retirement Notice to the Employee Benefits Division.
It is no longer necessary for the agency Benefits Administrator to call the Employee Benefits Division when an employee with M/C Life Insurance retires. In addition, the agency is no longer responsible for receiving the first direct monthly payment for M/C Life Insurance after the enrollee retirees.
Employee Benefits Division
Once the completed M/C Life Insurance Transition to Retirement Notice is received, additional information will be sent directly to enrollees continuing life insurance coverage. The information will include a direct pay billing notice, a supply of premium remittance notices for making payments directly to the NYS Department of Civil Service and information regarding payment through pension deductions.
Accident and Sickness Coverage
There are no conversion privileges for accident and sickness coverage. This coverage is not continued (not available to retirees) into retirement.
Enclosed is an initial supply of Management/Confidential Life Insurance Program Retirement forms PS-931, PS-932 and PS-915.1. To order more forms, complete the Employee Benefits Division Supply Request form (PS-458). If you need more forms immediately, please make photocopies from your current supply.
