The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

We are pleased to announce that the NYS Dental Plan Certificate of Insurance and Attachment to the NYS Dental Plan Certificate of Insurance documents for enrolled employees and their covered dependents are now available on HBA Online and NYSHIP Online. NYS groups with the NYS Dental Plan include: Council 82, M/C; Legislature, NYSCOPBA, NYSPIA, PBA Supervisors, PBA Troopers, PBANYS APSU, and PEF. The NYS Dental Certificate of Insurance is evidence of dental coverage under the group contract that the State of New York currently holds with EmblemHealth, the Plan’s administrator. The Attachment to the NYS Dental Plan Certificate of Insurance provides information about dental insurance benefits specific to each group.
Distribution
EmblemHealth will begin mailing these publications to enrollee homes starting in late summer/early fall. These documents will also be mailed to new enrollees joining the NYS Dental Plan throughout the year.
