SUBJECT: NYBEAS Dental and Vision Conversion
DATE: August 2, 2001

As a result of the recent NYBEAS conversion of enrollment records for the NYS administered Dental and Vision Programs, we have become aware that certain part-time and temporary employees may have been enrolled in these programs in error. Attached is a listing of these employees. Please review the listing to determine eligibility and enrollment for each employee and mark an "X" in the column to indicate the action to be taken. If the employee is no longer employed with your agency, mark a "T" and provide the date terminated, if available. When you have finished updating the list, return it to your processor no later than August 15th.

Mark an "X" or "T" in appropriate column.

Dental-No = not eligible or waived enrollment
Vision-No = not eligible or waived enrollment
Dental-Yes = eligible and is correctly enrolled
Vision-Yes = eligible and is correctly enrolled

The corrections in NYBEAS must be made by EBD staff, so that the updates to the GHI and Davis Vision records can be properly coordinated. DO NOT UPDATE NYBEAS YOURSELF! Each person on this list who received a GHI benefit card or Davis Vision booklet in error, will be notified that this error has been corrected, and that they are not eligible for GHI Dental and/or Davis Vision benefits.
