The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

SUBJECT: 2025 SEHP Benefit Card Distribution

In compliance with federal law, new benefit cards reflecting updated out-of-pocket costs for the 2025 plan year will be mailed to all SEHP enrollees and dependents on November 8. These cards have an expiration date of August 31, 2025 and replace cards distributed in July 2024.
The new cards do not need to be activated prior to use. Enrollees may begin using them immediately and should securely destroy all other cards.
The card file was pulled from NYBEAS on October 28, 2024. Any enrollee added to NYBEAS after that date will receive a card in the weekly card mailing for new SEHP enrollees.
